Recidief en esthetische resultaten bij basocellulair huidcarcinoom
Het recidiefpercentage en de esthetische resultaten bij de behandeling van basocellulaire huidkanker verschillen naargelang van de behandeling. Het recidiefpercentage is het laagst na chirurgie en radiotherapie. De esthetische resultaten zijn het best na fotodynamische therapie met aminolevulinezuur of methylaminolevulinezuur.
De auteurs hebben de gegevens doorgenomen van 40 gerandomiseerde studies en vijf niet-gerandomiseerde studies, die ze hadden teruggevonden in de literatuur tot mei 2018. Het recidiefpercentage werd berekend op grond van de gegevens over 2.204 tumoren. De esthetische resultaten werden geëvalueerd op grond van de gegevens over 739 tumoren. Meestal ging het om een oppervlakkig of een nodulair basocellulair carcinoom met een laag histologisch risico, de frequentste subtypes.
Het recidiefpercentage werd geraamd op 3,5% (95% betrouwbaarheidsinterval = 0,7% tot 16,8%) na radiotherapie, 3,8% (95% BI = 1,5% tot 9,5%) na excisie, 3,8% (95% BI = 0,7% tot 18,2%) na Mohs-chirurgie, 6,9% (95% BI = 4,6% tot 56,1%) na curettage en diathermie; 14,1% (95% BI = 5,4% tot 32,4%) na behandeling met imiquimod, 16,6% (95% BI = 7,5% tot 32,8%) na fotodynamische therapie met aminolevulinezuur, 18,8% (95% BI = 10,1%-32,5%) na fotodynamische therapie met methylaminolevulinezuur, 18,8% (95% BI = 10,1%-32,5%) na behandeling met fluoro-uracil, 19,9% (95% BI = 4,6%-56,1%) na curettage plus cryotherapie en 22,3% (95% BI = 10,2% tot 42,0%) na cryotherapie.
Het percentage patiënten met goede of zeer goede esthetische resultaten was 95,8% na fotodynamische therapie met aminolevulinezuur, 93,8% na fotodynamische therapie met methylaminolevulinezuur; 77,8% na excisie, 51,1% na cryotherapie en 20,0% na lasertherapie gevolgd door fotodynamische therapie.
